Another contribution from a MeG subscriber. Unfortunately, 
she didn't know the name of the poet though she did mention 
that this was sung by Tina Sani. So if anyone has any idea 
about the poet, please let me know.
 

chhoTii sii Galat-fahmii kar degii judaa ham ko
haalaat na badale.nge maaluum naa thaa ham ko
 
[Galat-fahmii = misunderstanding; haalaat = circumstances]
 

rishto.n kii hame.n itanii pahachaan na thii pahale
kahane ko maraasim to apane the bahut gehare
ashko.n ke sivaa lekin kuchh bhii na milaa ham ko
 
[maraasim = relationships; sivaa = other than]
 

uThate hue qadamo.n me.n zanjiir sii pa.D jaatii
ehasaas-e-nidaamat se dahaliiz par ga.D jaatii
ik baar muhabbat se dete jo sadaa ham ko
 
[ehasaase-e-nidaamat = feeling of repentance/regret]
[dahaliiz = threshold; sadaa = call]
